Blepharoplasty is one of those procedures whose results hold up for years, and for the upper eyelid often for a lifetime. Once recovery is complete and the swelling has settled, the excess skin and fatty tissue that were removed simply don't come back the same way - it's a lasting correction of the specific problem that existed at the time of surgery.
That doesn't mean the face freezes in time, of course. Skin keeps aging naturally afterward, so it's completely normal to notice some mild looseness again after a decade or more - this time as a result of new, later aging, not a return of the original problem. For upper eyelid surgery, this stretch of "clean" results tends to be quite long, often fifteen years or more. Lower eyelid surgery, which mainly addresses fat pockets, is repeated even less often in practice.
Exactly how long the result lasts depends on a few things - genetics, skin quality, age at the time of surgery, and everyday habits. Unprotected sun exposure and smoking can noticeably speed up the loss of skin elasticity, effectively eating into the result sooner than it would otherwise fade. That's why regular sun protection is considered one of the simplest ways to extend how fresh the look stays after surgery.
The final, mature appearance settles in gradually, over the weeks following the procedure, once the swelling has fully resolved.
